NOTE: Online scheduling may not work on all mobile devices. If you are scheduling a New Patient Visit your visit is not confirmed upon initial schedule and must be approved. If approved you will receive an email, if your request is denied you will receive a phone call with alternative dates of scheduling.

Cancellations and Rescheduling: You cannot cancel or reschedule an appointment online. Please email or call us directly to coordinate a schedule change.

New Patient Visit: We ask that you provide a credit card on file before we can approve your new patient visit request to reduce late cancellation and missing appointments. We have a 24 hour cancellation policy and you will not be charged if you cancel outside of that window. By scheduling you are agreeing to this cancellation policy and will be auto charged to card on file for 50% of the visit cost for the time scheduled for missed appointments.

You will be sent new patient paperwork via email. Paperwork must be completed 48 hours before your appointment or your appointment will be cancelled. You will receive a phone call or email if we are cancelling your appointment due to a paperwork issue to work to resolve that before losing your spot.
